Jedes Land verfügt über seine einzigartige Infrastruktur, die sowohl rechtliche Herausforderungen bei der Zulässigkeit von ausländischen Rezepten als auch technische Hürden bei der Einlösung elektronischer Verordnungen umfasst. In Deutschland ist die gematik für die Telematikinfrastruktur im Gesundheitswesen verantwortlich, während unsere Nachbarländer ihre eigenen Infrastrukturen und digitalen Gesundheitsdienstleister besitzen. Um die E-Rezepte der verschiedenen Länder einzulösen, wäre eine Schnittstelle zwischen ihnen notwendig. Bislang besteht diese Schnittstelle jedoch nicht zwischen Deutschland und seinen Grenzländern. Mit dem European Health Data Space (EHDS) soll dieser Sachverhalt sich ändern. Dieser gemeinsame EU-(und EWR-)Datenraum ermöglicht den sicheren und effizienten Gesundheitsdatenaustausch. Aktuell wird die digitale Plattform „MyHealth@EU“ errichtet, die die grenzüberschreitende Einlösung von E-Rezepten ermöglichen soll. Fast alle Nachbarländer sind dabei beteiligt, nur die Schweiz ist nicht. Bislang (Stand: zweites Quartal 2024) sind 14 europäische Länder an das System angeschlossen, darunter Luxemburg, Frankreich, Tschechien, Polen und die Niederlande.

Die Bedeutung einer sicheren Kommunikationskanale

Für Reisende innerhalb der EU soll „MyHealth@EU“ es ermöglichen, dieselbe Gesundheitsversorgung wie in ihrem Heimatland zu erhalten. Hierfür ist ein sicherer Kommunikationskanal zwischen den nationalen digitalen Gesundheitsdiensten erforderlich. Dieser Kanal stellt die wichtigsten Informationen in der Sprache des Reiselandes zur Verfügung. So können medizinische Fachkräfte in Krankenhäusern und Apotheken Patientenkurzakte in ihrer Amtssprache erhalten, die die bisherige Medikation, Vorerkrankungen und andere wichtige Gesundheitsdaten enthalten. Dadurch können individuell optimale Therapien und die Versorgung mit der im Heimatland verschriebenen Medikation gewährleistet werden. Ein digitaler Flyer, der das Verfahren aus verschiedenen Perspektiven zeigt, kann mehr Informationen liefern.

Die Umsetzung in den Apotheken

In einigen an „MyHealth@EU“ angeschlossenen Ländern sind bereits alle Apotheken über eine Schnittstelle zur Teilnahme an dem Dienst. In anderen Ländern können bislang nur in wenigen ausgewählten oder gar keine Apotheken grenzüberschreitende E-Rezepte eingelöst werden. Für die Umsetzung des „MyHealth@EU“-Services in den Apotheken müssen diese an die digitalen eHealth-Kanäle der EU angeschlossen werden. Dies geschieht über den National Contact Point for eHealth (NCPeH). Die Deutsche Verbindungsstelle Krankenversicherung Ausland (DVKA) des GKV-Spitzenverbands ist dafür zuständig. Die E-Patientenkurzakte und die E-Rezepte werden derzeit innerhalb eines bis 2025 laufenden Projektes getestet und sukzessive eingeführt. Voraussichtlich im ersten Quartal 2025 soll mit der Entwicklung des Einlöseweges von Deutschen E-Rezepten im EU-Ausland begonnen werden. Wann die Kanäle für die Belieferung ausländischer E-Rezepte in Deutschland bereitstehen, ist noch unklar. Die gematik berichtet, dass die grenzüberschreitende Belieferung von E-Rezepten binnen 48 Monaten nach Inkrafttreten der EU-Verordnung zur EHDS in allen Mitgliedsstaaten möglich sein muss. Das Inkrafttreten der Verordnung wird Anfang 2025 erwartet. Schneller, nämlich bis Ende 2025, soll der Aufbau von „MyHealth@EU“ in 25 EU-Ländern erfolgen. Welche zwei Länder in diesem Zeitplan außen vor bleiben, wird nicht offenbart.
