The research, presented at the 2025 American Academy of Allergy, Asthma, & Immunology (AAAAI) annual meeting, challenges existing paradigms in allergy treatment. By directly comparing omalizumab with oral immunotherapy, this study provides critical insights into which therapy offers better outcomes for patients with multiple food allergies.
The study involved 117 participants, predominantly young children, who were allergic to peanuts and at least two other common food allergens such as milk, eggs, or tree nuts. Each participant underwent an initial 8-week course of omalizumab injections. Following this phase, half of the group received oral immunotherapy while the other half was given a placebo.
By week 16, the trial transitioned into a double-blind phase where participants were randomized to either continue receiving omalizumab or a placebo injection for 44 weeks. The ultimate goal was to assess whether participants could tolerate significant amounts of their specific allergens—equivalent to approximately 20 peanuts or half a cup of milk—without adverse reactions.
Intent-to-treat analysis demonstrated that omalizumab was notably more effective than oral immunotherapy, with success rates of 36% compared to 19%. This difference was statistically significant, underscoring omalizumab’s superiority. However, when excluding dropouts, no significant differences emerged between the treatments.
The higher dropout rate among those receiving oral immunotherapy was primarily due to adverse events. Notably, 88% of the omalizumab group completed the trial without serious adverse reactions, while over 30% of the oral immunotherapy group experienced significant side effects, leading to discontinuation.
In the third stage of the study, 60 participants who successfully completed the earlier phases received an additional 24 weeks of omalizumab before discontinuing the medication. Early data revealed that many participants were able to introduce allergenic foods into their diets even after stopping omalizumab.
Consumption of milk, eggs, and wheat showed higher success rates (61% to 70%) compared to peanuts and tree nuts (38% to 56%). Adverse events like anaphylaxis or taste preferences influenced these outcomes. Despite some setbacks, the majority of participants maintained the ability to consume their allergens post-treatment.
These findings suggest that omalizumab could play a pivotal role in managing multiple food allergies, offering a safer and more effective alternative to oral immunotherapy. Clinicians now have a robust basis for recommending omalizumab, especially considering its favorable safety profile and higher success rates.
However, it is crucial to acknowledge the potential risks associated with any treatment. While omalizumab shows promise, ongoing research is necessary to fully understand its long-term benefits and limitations. For now, the study provides a strong foundation for advancing allergy treatment strategies and improving patient outcomes.
